If you require any vaccinations relating to foreign travel you need to complete the following Travel Risk Assessment Form. Please return this form to the practice at least 12 weeks before you travel.
From 28 May 2019 we require this form at least 3 MONTHS before you are travelling to allow our nursing staff to assess your needs and administer any necessary vaccinations for maximum efficacy.
It is the responsibility of the patient to ensure the form is received by the practice in a timely manner as failure to do this means that the practice cannot review the risk assessment form and administer the vaccinations.
The practice may charge for certain vaccinations so please check this when you drop off the Travel Risk Assessment Form.
