Enhanced Access Appointments 

The Practice offers a range of appointments for patients through Extended Access. Offering these appointments provides more availability for those who may struggle to get to the Practice during the week.  You may be offered an appointment with an Extended Access clinic when you call or you may ask to be booked into an Extended Access clinic if this better suits your needs.

Appointments are available between 6:30pm and 8:00pm from Monday to Friday, and from 9:00am to 2:00pm on Saturday. Theses appointments are available with a range of clinicians, including GPs, Nurses, Health Care Assistants and Phlebotomists.

Appointments are provided through our Primary Care Network and may take place at:

  • Queens Road Surgery – 252 Queens Road, Halifax HX1 4JG
  • Spring Hall Medical Centre – Spring Hall Lane, Halifax HX1 4JG
  • Boulevard Medical Centre – Savile Park Road, Halifax HX1 2ES

Appointments at Queens Road Surgery and Spring Hall Medical Centre are subject to parking control and patients must register their license number with reception. 

Cancer Care

Your GP is here to offer support following a cancer diagnosis.

Everyone’s experience of cancer is unique but there is a range of support services and information available to help you, your family and friends at this difficult time.

For non-medical concerns or worries it might be useful to ask your GP Practice to make you an appointment with a Social Prescriber or link worker who, if they can’t directly help, may be able to refer you to someone who can.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to complete the following Travel Risk Assessment Form. Please return this form to the practice at least 8 weeks before you travel.  

It is the responsibility of the patient to ensure the form is received by the practice in a timely manner as failure to do this means that the practice cannot review the risk assessment form and administer the vaccinations.

The practice may charge for certain vaccinations so please check this when you drop off the Travel Risk Assessment Form.

Private Examinations & Reports

A list of services not available on the NHS is displayed at the surgery with our current fees – for example insurance claim forms for private health care and holiday cancellation forms, letters of support, some travel vaccinations etc.

Please allow up to 21 working days for completion and remember you will have to pay the fee at the time the request is made.  Please make sure you have completed your part of the form before you bring it in

Dental Care

If you have a dental problem you need to visit your dentist. If you do not have a dentist, you should contact the local Dental Helpline on 01422 281576; for urgent dental treatment contact the Emergency Dental Service on 0845 122 0221. We do not deal with dental problems at the surgery.  

Smoking Cessation

Please contact Calderdale QUIT Smoking Service to arrange an appointment  01422 281505.

Minor Surgery

By appointment with your doctor for appropriate conditions, Friday afternoon 2.15 – 3.15pm. You must see your doctor for assessment before you are given an appointment.

Maternity Care

The initial assessment of your maternity care needs will be carried out by the midwife. If appropriate you will attend antenatal clinic with the midwife for regular checks. This clinic is no longer held at the Practice, your midwife will confirm the date, time and location of your antenatal appointment. From Tuesday 4th May pregnant women wishing to book their maternity care with Calderdale & Huddersfield NHS Foundation Trust will be directed to use their website: www.mypregnancynotes.com Follow this link and complete the form to create a log in. Once registered you will be contacted by a member of the team within 3 working days to arrange your first midwife appointment. Anyone who need further help or is unable to access the My Pregnancy Notes online can still contact the midwife team on 01422 2613351 or 01484 355731. Leave a message and a member of the team will be in touch within 3 working days. 

Family Planning

By appointment or arrangements with a practice nurse. Contraception services for all patients are given on a totally confidential basis.

Ear Irrigation

We no longer offer this service. Please read the leaflet below which explains how to take care of your ears and what to do to disperse a build up of wax. If your ear(s) still feel blocked after 4 weeks of following this self-care, please call us for an appointment to have your ear(s) assessed.
