New Patient Registration
As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and if you take regular medication, you will be offered a consultation with the Health Care Assistant.
The process of registration takes up to 10 working days. Medical treatment is available from the date of registration. Please contact reception for further information.
Check you live in our catchment area using our interactive map.
This includes Friendly, Warley, Highroad Well, Norton Tower, Willowfield, Trimmingham, Pellon, Savile Park, Skircoat Green & Copley.
Please inform us of changes of address, telephone number or name. If you move outside our practice area you may have to change doctors. See NHS Choices for your nearest GP Service.
During the COVID-19 situation our Registration Form and New Patient Health Questionnaire is available online. Please complete both forms once and press send. Please do not send multiple forms.
Please complete the online forms below, not completing some sections may result in your registration being delayed or rejected.
Please be aware that:
- You should only send this form if you are sure that you are eligible to join this practice. Registrations are currently being accepted for those who have moved into the area and who are not already registered with a local GP surgery.
- Sending this form will not automatically register you with the surgery.
- Your details will be held at the surgery for a limited period of time. You are required to present in person to sign your registration form and provide proof of your address once the Practice returns to normal.
- Sending this form does not guarantee or even imply that you will be accepted onto the practice register.
Once your registration has been processed we will send you a health questionnaire to be completed in order to complete your registration with the Practice.
